SUMMARY OF CRASH: On 11/13/2017 at approximately 1350 hours, the Vermont State

Police were notified of a two vehicle crash that occurred on North Derby Road in the

Town of Derby, Vermont. The complainant, Provoncha, reported the other operator had

taken off from the scene. A unit from Orleans County Sheriff’s Department and a trooper

located the vehicle and identified the operator as Cynthia Smith. Investigation revealed

Provoncha, operating the TT-Unit, was backing into a parking lot when Smith attempted to

overtake the TT-Unit, passing on the right. Investigation revealed Smith struck the front

bumper of the TT-Unit and continued traveling south on North Derby Road. Crash is currently

still under investigation.
