SUMMARY OF CRASH: On 5/21/16 at approximately 1100 hours VSP responded to a
report of a vehicle vs bicyclist at the intersection of Sunderland Hill Rd. and
Bacon Hollow Rd. within the Town of Sunderland.  Through statements and
investigation it was learned that Op#1 was traveling south on Sunderalnd Hill
Rd. when she turned left in front of OHara who was traveling north on Sunderland
Hill Rd.  OHara steered to the right but was unable to avoid V#1.  OHara
collided with V#1 passenger side area.  OHara was thrown from the bike.

OHara was transported to SVMC by Arlington Rescue for back injures.  Op#1 was
not injured.  Op#1 was issued a ticket for not yielding Right of Way.
